Non-GP appointments can be booked by calling the surgery on 01793 614840. 

If you require an interpreter or a home visit please call the surgery to make this request. 

How to book a GP appointment

Request a GP Appointment 

Upon clicking the link above you will be directed to our Rapid Health page. You will be asked to enter your details and to enter information regarding your medical condition. 

You will then be guided through an online form which allows us to triage your request to ensure you are seen by the right clinician in the correct time frame. 

Please be advised if you are unable to complete the form online a member of our Care Navigation Team can assist you in completing this form over the phone. This process will take longer than an average phone call and we are unable to override the system to book you an appointment without the completion of this form. 

If you have a family, friends or a carer, who can support they will be able to complete the form on your behalf.

If you require an interpreter for your appointment, please submit an admin request using the link below. Please do not complete the triage form online, submitting an admin request allows us to assist you more appropriately. Choose 'Ask a question' to submit your request.

To book a Long Term Condition Review or a routine nurse appointment (for cervical screening, wound care or HRT review), please continue to call us on 01793 614840 — we're working behind the scenes to bring a direct booking option (coming soon).

Booking an Out of Hours Appointment

We appreciate that some our patients may need to be seen or spoken to outside of our normal hours 08:00 to 18:30. 

To help accommodate this need we have Saturday appointments available through our Saturday Hub as well as access to a GP via the Livi App.

Admin Requests

Our Rapid Health System also allows our patients submit requests related to admin needs. This may include: 

  • Updating your details 
  • A query regarding your medication 
  • Submission of a request for private GP work such as a letter.

Accurate Information Entry: When scheduling appointments through the Livi App, it’s important for you to ensure that all the information you input is accurate. This includes all personal details including full name, date of birth, and home address that match to this information held by your GP practice. Accuracy is vital to avoid any discrepancies that could potentially lead to appointment cancellations or delays in your care.

Timely Cancellation: Please be mindful of the importance of promptly cancelling any Livi appointments made through the app if you no longer require them. By doing so, you can free up appointment slots for other patients in need of assistance. This will help optimise our scheduling system and ensure that everyone receives timely care.
