Or visit our front desk reception to collect a registration pack, once completed please return it to our reception desk along with 1 form of photographic identification (passport, driving license etc) and a utility bill (produced within the past 6 months) to confirm your address

  • Completed New Patient Questionnaires for each person (these can also be completed in the surgery)

  • Completed GMS1 Registration Form (these can also be completed in the surgery) – it is important that these forms are completed fully and clearly

  • Registration for online appointment booking can be provided. This allows you to make appointment online and receive reminders of any appointments with the GP or Nurse. Please ask at reception for more information.

  • Patients who move to an address which is not within our catchment area will unfortunately not be able to remain registered with the practice and will be asked to register with a local GP.

Please allow 2 weeks for the registration to process, if your registration is more urgent, please inform the Receptionist who will endeavour to register you more promptly.

Registering online as a non-English speaker

You can find guidance and advice on how to become a new patient and the NHS healthcare services you can access, if your first language is not English, or if you’re an asylum seeker on the Government website.

Registering as a temporary patient

If you are away from home, you can register as a temporary patient with Ashington House Surgery for up to three months. This will allow you to use our surgery and still remain a patient at your permanent GP surgery. After three months, you will have to re-register as a temporary patient or permanently register with us.

Registering children

Children under 16 need to be registered by their parent or guardian. You can do this by completing the online GP surgery registration form on their behalf. You will need to provide a form of ID to prove that you’re their parent or guardian.

What to do if you’re registering on behalf of someone else

If you are a carer of young children, elderly adults or adults who aren’t able to register themselves, you can set up an account on their behalf. This will allow you to book an appointment and access parts of their GP online services account.

Change of address

If you are moving house, please let us know as soon as possible. If you are moving out of the surgery’s catchment area, you may be asked to register with a surgery that’s closer to your new home address.

You can contact us online to let us know about a change in your address.

Please also let us know if your telephone number or email address changes.

It would be helpful if you could register Monday – Friday afternoons from 2.00pm.
