Long Term Conditions Coordinator.

The purpose of this review is to allow you the opportunity to discuss with a clinician how you are managing your long term condition and to ensure that any medication you are taking for your condition is being safely prescribed and is effective. During your review you our clinician can answer any questions you have about your condition and we can offer you advice and guidance. Your review will often involve us reviewing your blood pressure and ensuring your height and weight are up to date in your records. You may also require a blood test in your review.

Ahead of your review our LTC Coordinator may be in touch with you to ask you to complete a questionnaire before your appointment. These questionnaires are used to ensure that you are able to get the most out of your appointment and allows our clinical team to assess how you are managing ahead of your appointment. If you are struggling to attend your long term condition review or are unable to complete the pre appointment questionnaires you have received please get in touch with our LTC Coordinator and they can offer you support.
